LHA Club Admin Forms for 2014-15 Season

A number of the Club Administration Forms have been updated prior to the start of the season and information has been distributed to all clubs and schools were applicable.
Please note the following :
1. Player Transfer Form (Click Here) - this form should be completed for the transfer of any player within the Leinster Hockey Association with immediate effect
2. Code of Conduct (Click Here) The LHA Code of Conduct has been updated and it is now a requirement for every club to return a copy which should be signed by the Hon Secretary to admin@leinsterhockey.ie or 28 Charlestown Avenue, St Margarets Road, Dublin 11.
3. Match Cards (Click Here) A soft copy of the match card can be downloaded from the website.
4. Text messaging prompt sheet (Click Here) this form has been updated. Results officials are reminded that for Cup Games the final result must be returned. For example if the score is a draw at the end of normal time and the game goes to extra time or penalties, the result of the extra time or penalties (which ever is final) should be returned.
5. Match times - Each club is responsible for ensuring that match times are correct. These should be updated on the website on the Monday before the game. Login's and passwords are the same as last season. Access to the Admin Login is on the homepage of the website or via the About Us/Admin Area on the site.
