New Garda Vetting Form

The Irish Hockey Association has advised that a new Garda Vetting form has been 
released by the Central Vetting Unit for immediate use for all vetting applications from today (Fri Jun 20th).
This form is available to dowload now and should be used for all future applications. Applications received on the old form will not be processed by the GCVU and will be returned.
