Under 16 Boys Interprovincial Nominations

Clubs and Schools are requested to submit their nominations for the Boys Under 16 Interprovincial Trials. Players must be under 16 by 31st December 2013.
Please note that nominees should be of /or close to Under 16 Interprovincial Standard. Close off date for receipt of nominations is Monday, 20th May.
When submitting names please ensure to include ALL the following information :
Player Name
Player DOB
Parent/Guardian Name
Parent/Guardian Contact Number (mobile)
Parent/Guardian Email Address
School/Club
Indicate if Goal Keeper
